    What Are Analgesics?

    An analgesic (also known as a painkiller) is any member of the group of drugs used to relieve pain (achieve analgesia). The word analgesic derives from Greek an- (“without”) and algos (“pain”). Analgesic drugs act in various ways on the peripheral and central nervous systems;. The pain relief induced by analgesics occurs either by blocking pain signals going to the brain or by interfering with the brain’s interpretation of the signals, without producing anesthesia or loss of consciousness. There are basically two kinds of analgesics: non-narcotics and narcotics.

    . Analgesia—condition where there is relief of pain without loss of consciousness.

    Analgesics—agents which cause analgesia.

    Narcosis—a condition of CNS depression: Narcosis, the unconsciousness induced by a narcotic drug

    Inflamation– A basic way in which the body reacts to infection, irritation or other injury, the key feature being redness, warmth, swelling and pain.

    Anti-Inflammatory drugs--

    Nonsteroidal anti-inflammatory drugs, usually abbreviated to NSAIDs or NAIDs, are drugs with analgesic and antipyretic (fever-reducing) effects and which have, in higher doses, anti-inflammatory effects (reducing inflammation)

    Classification of the Anti-Inflammatory drugs—

    NSAID—non steroidal anti-inflammatory drugs (Aspirin)

    SAID—steroidal anti-inflammatory drugs (Corticosteroids)

    SAARD—slow acting anti-rheumatic drugs

    DMARD—disease modifying anti-rheumatic drugs (Methotrexate)

    Basically Analgesic drugs can be divided in two type –

    A- Non-Narcotic

    B-Narcotics

    Non-Narcotic Analgesics

    Acetaminophen is the most commonly used over-the-counter, non-narcotic analgesic. Acetaminophen is a popular pain-reliever because it is both effective for mild to moderate pain relief and relatively inexpensive. It must be emphasized though that the safety of acetaminophen is tied to proper use of the drug (use according to specific prescribing instructions). If acetaminophen is not used according to the directions on the label, serious side effects and possible fatal consequences can occur. For example, taking more than 4000 mg/day or using it longterm can increase the risk of liver damage. The risk of liver damage with acetaminophen use is also increased by ingesting alcohol. Make sure you discuss with your doctor the maximum allowable dose of acetaminophen and any other guidelines for its use.

    Many people do not realize that acetaminophen is found in more than 600 over-the-counter drugs. It can be found in combination with other active ingredients in many cold, sinus, and cough medications. The cumulative effect of acetaminophen must be considered if you are talking multiple drugs which contain acetaminophen.

    How can acetaminophen damage the liver? Acetaminophen changes into metabolites which are eliminated from the body. By taking more than the recommended maximum daily dose of acetaminophen, more toxic metabolites are produced than can be eliminated.

    Narcotic Analgesics

    There are two types of narcotic analgesics: the opiates and the opioids (derivatives of opiates). Opiates are the alkaloids found in opium (a white liquid extract of unripe seeds of the poppy plant).

    Opioids are any medication which bind to opioid receptors in the central nervous system or gastointestinal tract. According to Wikipedia, there are four broad classes of opioids:


    • Endogenous opioid peptides (produced in the body: endorphins, dynorphins, enkephalins)

    • Opium alkaloids (morphine, codeine, thebaine)

    • Semi-synthetic opioids (heroin, oxycodone, hydrocodone, dihydrocodeine,

    1-Fugu (blow fish) is a fish which contains deadly poison in the organs. Despite the risk, fugu dishes remain as special feasts in Japan. Even the milt is considered as a great delicacy. Fugu is referred to as blow/globe/puffer fish since it blows its body up. The kanji (Chinese characters) used to write fugu indicates “river pig.” In western Japan, fugu is called “fuku,” which means “to blow” or “happiness.”

    2-Shellfish

    In shellfish poisoning, the poisonous ingredients are toxins made by algae-like organisms called dinoflagellates, which build up in some types of seafood. There are many different types of shellfish poisoning. The most well known types are paralytic shellfish poisoning, neurotoxic shellfish poisoning, and amnestic shellfish poisoning.


    Shellfish poisoning is mostly associated with bivalve mollusks (mussels, clams, oysters, and scallops). These shellfish are filter feeders and can accumulate high levels of toxics including domoic acid, produced by microscopic algae. Cooking doesn’t always inactivate these toxins and some can lead to death in high dosages. Kosher diets anyone?

    3-Rhubarb

    The leaves of the plant contain poisonous substances such as oxalic acid, which is a nephrotoxic and corrosive acid. The oxalic acid content of the plant may vary, though the typical value is around 0.5%. Its leaves should not be cooked with soda, which increases its poison content


    It is believed that another unidentified toxin is present in the leaves. It might be anthraquinone glycoside, also known as senna glycosides. It also makes the consumption of rhubarb leaves harmful.


    Ever wonder why Rhubarb recipes mostly contain the stem and not the leaves? The leaves contain higher concentrations of oxalate, which causes poisoning in large doses. The stems contain much less of the compound. Anthraquinone glycosides are also present and may add to its toxicity.

    4-Potatoes

    A bitter poisonous glyco-alkaloid, C45H73NO15, derived from potato sprouts. It can occur naturally in the any part of the plant, including the leaves, fruit, and tubers. It is very toxic even in small quantities. Solanine has both fungicidal and pesticidal properties, and it is one of the plant’s natural defenses. Potatoes naturally produce solanine and chaconine, a related glycoalkaloid, as a defense mechanism against insects, disease, and predators. Potato leaves and stems are naturally high in glycoalkaloids.

    Solanine poisoning is primarily displayed by gastrointestinal and neurological disorders. Symptoms include nausea, diarrhoea, vomiting, stomach cramps, burning of the throat, headaches and dizziness. Hallucinations, loss of sensation, and paralysis, fever, jaundice, dilated pupils and hypothermia have been reported in more severe cases.In large quantities, solanine poisoning can cause death. One study suggests that doses of 2 to 5 mg per kilogram of body weight can cause toxic symptoms, and doses of 3 to 6 mg per kilogram of body weight can be fatal.Symptoms usually occur 8 to 12 hours after ingestion, but may occur as rapidly as 30 minutes after eating high-solanine foods.

    Most solanine occurs in the skin or just under the skin of potatoes. Peeled potatoes have been found to contain 30-80% less solanine than unpeeled potatoes, and green potatoes should always be peeled if they are to be used at all. Solanine and chaconine are also present in potato shoots.

    Deep-frying potatoes at 170 °C is effective at lowering glycoalkaloid levels, boiling is ineffective, and microwaving only somewhat effective.

    5-Apricot Stones (Seeds)

    Actually, many fruits of the rose family contain cyanogenetic glycosides, which upon ingestion release hydrogen cyanide gas. These include seeds from cherries, apples, plums, almonds, and peaches. Although it would take a mega-dose to do you in, keep this in mind the next time you go on an apricot seed binge. There have been reported deaths in Turkey, a large apricot grower, for this very reason.

    6-Tomato.


    Although the fruit itself is safe (more technically a berry), tomato plants contain glycoalkaloids, which are indeed toxic.

    7-Chocolate.


    Chocolate contains the alkaloid theobromine, which in unusually high doses can be toxic to humans. However, it would take an unholy amount of chocolate to achieve this. But other species react much worse: dogs, parrots, horses, and cats can be killed by ingesting chocolate.

    Theobromine poisoning or chocolate poisoning is an adverse reaction to the alkaloid theobromine, found in chocolate, tea, cola beverages, and some other foods. Cacao beans contain about 1.2% theobromine by weight, while processed chocolate generally has smaller amounts. The amount found in highly refined chocolate candies (typically 40-60 milligrams per ounce or 1.4 to 2.1 grams per kilogram) is much lower than that of dark chocolate or unsweetened baker’s chocolate (over 400 mg/oz or 14 g/kg).

    The amount of theobromine found in chocolate is small enough that chocolate can be safely consumed by humans in large quantities, but animals that metabolize theobromine more slowly can easily consume enough chocolate to cause chocolate poisoning. The most common victims of theobromine poisoning are dogs (for which it can be fatal). Cats and especially kittens are yet more sensitive, and many other animals are also susceptible.

    The first signs of theobromine poisoning are nausea, vomiting, diarrhea, and increased urination. These can progress to cardiac arrhythmias, epileptic seizures, internal bleeding, heart attacks, and eventually death.Theobromine is especially toxic to horses, dogs, parrots, voles, and cats because they are unable to metabolize the chemical effectively. If they are fed chocolate, the theobromine will remain in their bloodstream for up to 20 hours. Medical treatment involves inducing vomiting within two hours of ingestion, or contacting a veterinarian.

    8-Tapioca.

    Tapioca, if not processed properly, can be toxic. It contains a natural source of cyanide, whose precursor, linamarin, is rendered harmless if properly dried, soaked, and baked.

    9-Peanuts.

    Our little top hat and spectacle friends can be deadly for those allergic, which isn’t all that uncommon. Watch out for artificially flavored nuts as well; they can be re-flavored with another kind of nut that you might be allergic to.

    10-Mushrooms.

    Most people know that certain types of mushrooms can be deadly. The more common poisonings come from the Death Cap (often confused for the Paddy Straw mushroom), Destroying Angels, and Deadly Webcap. Death Caps alone contain over seven toxins and one bite can lead to a nasty death. Watch out for the Gyomitra, which is often confused for Morrel.

    BAKTWANG, India — The head of the family and leader of the “Chana” sect — which allows polygamy — is 66-year-old Ziona Chana Mr Chan belongs to a sect where he can marry as many times as he wants and infact he is the head of that sect too.who is the ‘proud’ head of a jumbo sized family of 39 wives, 94 children, 14-daughters-in-laws and 33 grandchildren meaning its entire 181 members, this family is the World’s biggest family - setting the world record
    But when he was growing up, Ziona believed he would never marry, saying: “My father had seven wives and looking after them was a difficult task. When I saw him surrounded by women all the time it put me off. But my wish was not God’s wish.”
    He even married ten women in one year, when he was at his most prolific

    Joint family – an Indian concept prevalent among Hindus is not commonly followed in Northeast India. In this Christian dominated part of the country, people are usually more educated and ‘westernized’. But Ziona Chana from Mizoram thinks ‘differently’. The Ziona family lives in a 100-room mansion, a 4 storey building situated at the hills of Baktwang village in the Indian state of Mizoram
    It almost looks like a hotel.
    The family’s home is called Chhuanthar Run – which translates as “the house of the new generation” – in the hills of Baktawng village, in the Indian state of Mizoram.
    It has its own school, a playground, carpentry workshops, piggery and poultry farms, paddy fields – and a vegetable garden big enough to feed the entire extended family.
    Chana thanks God for giving him a huge family to look after and considers himself ‘lucky.’
    Mr Chana’s grandfather also had many wives and according to one of his sons,his father marries the poor women from the village so he can look after them.
    All the men of the family work as carpenters and Ziona says proudly: “We don’t want any help from the government.

    Family Management
    The family is a very well organised one living a disciplined life with the oldest member of the family giving the orders and mutually cordinating with each other in household chores such as cleaning, washing and preparing meals.The whole family system is based on ‘mutual love and respect’
    Meal times are like feeding an army, with the women of the family spending hours making dinner.
    A typical meal can see them plucking 30 chickens, peeling 132lb of spuds and boiling up 220lb of rice.
    All the cooking is done over an open fire kept burning throughout the day.
    Ziona’s eldest wife Zathiangi, 69, delegates all the daily duties around the house to his other wives, their daughters and the daughters-in-law.
    He keeps the youngest women near to his bedroom with the older members of the family sleeping further away – and there is a rotation system for who visits Mr Chana’s bedroom.
    He sleeps in his own double-bedded room while his wives have to adjust in dormitories.
    Surprisingly, all the wives get along very well and there is hardly any infighting among them.

    Rinkmini, one of Mr Chana’s wives who is 35 years old, said: ‘We stay around him as he is the most important person in the house. He is the most handsome person in the village.
    She says Mr Chana noticed her on a morning walk in the village 18 years ago and wrote her a letter asking for her hand in marriage.
    Mr Chan,the head of the family told in an interview:
    ‘Today I feel like God’s special child. He’s given me so many people to look after.
    ‘I consider myself a lucky man to be the husband of 39 women and head of the world’s largest family.’
    Surprisingly all his wives are quite happy with each other and have no problem at all.
    Mr Zionamarried as many as 10 times in a year once and his wives take it in turns to share his bed.The youngest wife sleeps near to his bedroom with the olderwives sleeping further away.
    One of her wife,Rinkmini, 35 who he spotted on a morning walk in the village 18 years agoand later sent the proposal, told:
    ‘We stay around him as he is the most important person in the house. He is the most handsome person in the village.

    ‘To expand my sect, I am willing to go even to the U.S. to marry,’

    Diabetes Mellitus is a chronic metabolic disorder that prevents the body from utilizing glucose completely or partially. It is characterized by raised glucose concentration in the blood. The pancreas produce a hormone insulin which controls the amount of glucose in the blood. Without insulin, the blood sugar increases as the body cannot use or store glucose. The most commonly-used screening tests are the determination of the fasting blood glucose level and the blood glucose level tested two hours after a meal. The normal fasting blood sugar content is 80-120 mg per 100 ml of blood; this can go up to a level of 160 mg per 100 ml of blood two hours after meals. Anything above these levels can be termed as diabetic levels. Diabetes is common among older, obese people.

    Types of Diabetes:
    Type 1 or insulin-dependent diabetes mellitus (IDDM) occurs when the pancreas fails to make enough insulin. It usually occurs in childhood or adolescence but can develop at any age. These patients need insulin everyday.


    Type 2
    or non-insulin dependant diabetes mellitus occurs when body cells become resistant to insulin. This is more common among adults, especially those who are overweight and over age 40. These people are able to control their blood sugar levels through weight control, regular exercise and a well-balanced diet. Some people require oral medications or insulin injection to lower blood sugar.

    Symptoms of Diabetes

    Initial Symptoms

    Increased thirst, increased urination, increased appetite are the initial symptoms.

    In type 1 weight loss is seen and in type 2 obesity is seen.

    Other Symptoms

    Other possible symptoms are blurred vision, skin irritation or infection, weakness or loss of strength, and decreased healing capacity.

    Clinical lab test data reveal glycosuria (sugar in urine), hyperglycaemia, abnormal glucose tolerance tests.

    If diabetes is not diagnosed at the right time symptoms like fluid and electrolyte imbalance, acidosis, coma is seen.

    There are approximately 400 plants used as diabetic remedies.

    - In ancient times high-fiber wheat grains were used to treat diabetes.

    - In recent time plant food have confirmed the effectiveness in treating diabetes.

    - Asia, Europe, Middle East have been using garlic, raw onions as anti-diabetic since long.

    - Europeans use common mushroom; people of Iraq use barley bread

    - Some countries have been using bitter gourd and herb ginseng as anti-diabetic herb.

    Blueberry, fenugreek seeds, coriander seeds, beans, Gooseberry, cucumber, lettuce, cinnamon, cabbage etc are used in treatment of diabetes in different countries

    Bitter gourd

    Bitter gourd widely found in Asia and Africa is popular for its medicinal properties and is recommended for diabetics.

    Bitter Melon, also known as Karela, Momordica Charantia or Bitter Gourd is a herb that helps regulate blood sugar levels and keeps body functions operating normally.

    It contains Gurmarin, a polypeptide considered to be similar to bovine insulin, which has been shown in experimental studies to achieve a positive sugar regulating effect by suppressing the neural response to sweet taste stimuli.

    Bitter melon also contains a lectin that has insulin-like activity due to its non-protein-specific linking together to insulin receptors. This lectin lowers blood glucose concentrations by acting on peripheral tissues and, similar to insulin’s effects in the brain, suppressing appetite. This lectin is likely a major contributor to the hypoglycemic effect that develops after eating bitter melon.

    Philippine Department of Health determined that a daily dose of 100 mg per kilogram of body weight is comparable to 2.5 mg/kg of the anti-diabetes drug glibenclamide taken twice per day.Tablets of bitter melon extract are sold in the Philippines as a food supplement under the trade name Charantia and exported to many countries.



    It has lately been established that bitter gourd contains a hypoglycaemic or insulin-like principle, designated as ‘plantinsulin’, which has been found valuable in lowering the blood and urine sugar levels. It should, therefore, be included liberally in the diet of the diabetic. For better results, the diabetic should take the juice of about four or five bitter gourd every morning on an empty stomach. The seeds can be added to food in a powdered form. Diabetics can also use bitter gourd in the form of a decoction by boiling the pieces in water or in the form of dry powder.





    Jambul Fruit

    Black berry fruit is another effective home remedy. It is regarded in traditional medicine as a specific against diabetes because of its effect on the pancreas. The fruit as such, the seeds, and fruit juice are all useful in the treatment of this disease. The seeds contain a glucoside ‘jamboline’ which is believed to have the power to check the pathological conversion of starch into sugar in cases of increased production of glucose. The seeds should be dried and powdered. One teaspoon of this powder should be mixed in one cup of milk or water or half a cup of curd, and taken twice daily. The inner bark of the black berry tree is also used in the treatment of diabetes. The bark is dried and burnt. It will produce an ash of white colour. This ash should be pestled in mortar, strained and bottled. The diabetic patient should be given ten grams of this ash on an empty stomach with water in the morning, and twenty grams in the afternoon, and in the evening an hour after taking meals. An equal quantity of indian gooseberry powder, jamun powder, and bitter gourd powder also makes a very useful remedy for diabetes. A teaspoon of this mixture once or twice a day would be effective in checking the progress of the disease.

    Grapefruit

    Grapefruit is a splendid food in the diet of a diabetic patient. If grapefruits were eaten more liberally, there would be much less diabetes. If you have sugar, use three grapefruits three times a day. If you do not have sugar, but a tendency towards it and want to prevent it, use three a day.

    Fenugreek

    The seeds of fenugreek have been found effective in the treatment of diabetes. Fenugreek seeds, when given in varying doses of 25 gm to 100 gm daily, diminish reactive hyperglycaemia in diabetic patients. Levels of glucose, serum cholesterol, and triglycerides were also significantly reduced in the diabetes patients when the seeds were consumed.

    Bengal Gram

    Experiments have shown that the intake of water extract of Chickpea enhances the utilization of glucose in both diabetic and normal persons. When kept on a diet which included liberal supplements of Chickpea extract, the condition of tile patient improved considerably and his insulin requirement was reduced to about twenty units per day. Diabetes patients who are on a prescribed diet which does not severely restrict the intake of carbohydrates, but includes liberal amounts of Chickpea extract, have shown considerable improvement in their fasting blood sugar levels, glucose tolerance, urinary excretion of sugar, and general condition.

    Black Gram

    For a milder type of diabetes, two tablespoons of germinated black gram, taken with half a cup of fresh bitter gourd juice and a teaspoon of honey, is said to be useful. It should be taken once daily for three to four months. A restriction should be placed on the intake of carbohydrates. Even in severe cases, regular use of this combination, with other precautions, is useful as a health-giving food for the prevention of various complications that may arise due to malnutrition in diabetics.

    Mango Leaves

    The tender leaves of the mango tree are considered useful in diabetes. An infusion is prepared by soaking 15 gm of fresh leaves in 250 ml of water overnight, and squeezing them well in the water in the moming. This filtrate should be taken every morning to control early diabetes. As an alternative, the leaves should be dried in the shade, powdered and preserved for use when necessary. Half a teaspoon of this powder should be taken twice a day.

    Parslane

    The seeds of parslane are useful in diabetes. A teaspoon of the seeds should be taken every day with half a cup of water for three to four months. It will increase the body’s own insulin and help in curing diabetes.

    Diabetes treatment using Other Foods

    Besides bitter gourd, certain other vegetables have been found useful in diabetes. These include string beans, cucumber, onion, and garlic. Tea made of the pods of string beans is valuable in diabetes.



    Oats are known for the anti oxidants and they help in achieving stability in blood sugar.

    Onion and garlic are also recommended as part of regular nutritional diets for diabetics.





    Appple

    - Apple is one of the most valuable sub-acid fruit. Pectin found in apples is known to help fight diabetes and apples decrease the body’s insulin needs.

    - It is rich in pectin content; which is a natural therapeutic ingredient and hence extremely beneficial in the treatment of diabetes.

    - Pectin is found in the inner portion of rind and pulp.

    - It helps in detoxification of the body and eliminates harmful substances.

    - Apple helps reduce 35 % of the body’s insulin requirements.

    As far as medicine is concerned, it reduces the blood sugar but not the symptoms and cause, so dose of medicine has to be increased day by day,while apple reduces the blood sugar as well as symptoms and cause,[not permenatly] and so there are no side effects of Diabetes. Besides, apple has fructose which protects automatically from going to hypoglycemia, unlike other various remidies and medicine.On Diabetes apple gives little delayed results I found that Apple has a unique inherent quality of reducing blood sugar,

    According to one patient when sugar is high and if he takes apple it will bring down blood sugar level immediately within 25 to 30 minutes. with this result now he is not afraid of high sugar levels even of it is 250 too 300 mg / dl.because as soon as he takes apple and walk for 5 to10 minutes sugar level topples down by 60 to 90 mg /dl per hr. So he made it a point to take at least one apple daily and je is able to keep his normal sugar level and fun is, it does not allow to go down lesser than 80 to 90 mg / dl and keeps it at 100 to 120 mg / dl all the time. I think it may be because of Fructose in Apple.





    Artichoke



    The insulin in artichoke is effective for the treatment of diabetes.

    - Artichoke is a tuberous root and its top resembles a sunflower.

    - It contains Potassium, calcium, iron and sulphur required for maintaining good health.

    - Artichoke is high in insulin content and thereby beneficial in the treatment of diabetes.

    - About 2 % of insulin is found in fully ripe artichoke.

    - Ripe artichoke is available only during autumn.

    - Artichoke are very effective when they are eaten in their raw form especially salads.

    - They can even be cooked with other vegetables.

    - For cooking boil the unpeeled artichoke in small quantity of water for 10 minutes.

    Dietary Guidelines for Diabetes Management

    Three methods of treatment are generally followed depending on the individual and severity of the conditions.

    1) Diet alone

    2) Diet and Oral hypoglycaemic drugs and

    3) Diet & insulin.


    A mild diabetic can manage his disease with control on his diet alone. A severe diabetic requires dietary control along with insulin. However, whatever the form of controlling diabetes, regularity, routine and regulation should be strictly followed.

    Types for Foods to be eaten for Diabetics

    A diabetic should follow a low carbohydrates, low fat & moderate protein diet. The diet should be high in complex carbohydrates ie. High fibre diet like vegetables, whole legumes, whole wheat products, oats etc. Products made from refined wheat flours should be restricted. Salads can be consumed in a any quantities but high fat salad dressings should be avoided. Fruits can be taken as per recommended by your dietician, fruit juices should be avoided and whenever possible fruit should be taken with the skin eg. Apple, Pear etc. Preferably avoid fruits along with your meals and you can take it 2 hours after or before a meal.

    Each meal should contain some source of protein as it doesn’t increase the blood sugar immediately

    For eg. In breakfast you can consume oats porridge, which contains oats as well as milk. Proteins should not be taken too much in excess as it may lead to renal problems.

    A diabetic patient should worry about the fat content of his diet. Many foods have some amount of invisible fats such as whole milk, egg yolk, and red meats.

    Choose skimmed milk, egg whites, de-skinned chicken and use very little quantity of visible fats like oil, Clarified butter, butter in your daily diet.
